The following are required to be submitted in order for a business license to be issued:

A. Application Form: A completed general business license application form.

B. Bond: A surety bond in the amount of twenty thousand dollars ($20,000.00) and subject to the conditions as described in section 3-1-14 of this title.

C. Supplier Information: The business name and address of each factor supplying premiums, merchandise or service with which it will do business; or, if applying as a factor, the same information for each telemarketing business with which it will do business.

D. Copies Of Materials: A copy of all sales presentations, scripts, advertising bulletins, and all other promotional materials used by the applicant and/or solicitors in the process of doing business.

E. Description Of Items: A complete and accurate description of each item offered as merchandise or premiums. Include a photograph or accurate sample of each item. The description must be the same as that given to the prospective purchaser or recipient.

F. Certifications:

1. Certification from the chief of police that applicant, its principals and managing employees have not been convicted of theft, fraud, obtaining money under false pretenses, embezzlement or other crime involving moral turpitude.

2. Certificate from the applicant that there are no adverse judgments against its principals or managing employees in civil actions involving allegations of fraudulent advertising, sales or trade practices or breach of contract other than as set forth in the certificate, together with a detailed explanation of circumstances, and certifying further that there has been no permanent injunction against any of said parties arising out of an adjudicated civil action that would prohibit them from engaging in fraudulent advertising, sales or trade practices in any jurisdiction.
