Wherever in this title there is a requirement for review of the criminal history of an applicant for a license or an employee of such applicant, the consent and authorization of such individual for a criminal history check shall be provided on an authorization form supplied by the city attorney. After request by the license officer, no license, permit, certification or approval shall thereafter be issued until such consent and authorization is provided and the criminal history check is completed, and the individual shall be so advised. A reasonable fee may be charged by the police department for checking criminal history, and such amount shall be remitted along with the authorization provided for herein. Any individual shall have the right to refuse such authorization, but in that event the city shall not be required to issue the license, permit or certificate requested by the individual or his employer.
